Targeted Care Packages (TCPs) are an allocation of funding attached to a specific child or young person which are used to prevent the child or young people from entering residential care, or transition from residential care into an alternate living arrangement that better meets their needs.

As a Key Worker you will provide individualised and flexible case management support to children and young people who present with challenging behaviours and are supported by a TCP. You will be supporting children, young people and their carers across a range of individualised placement options including foster care, kinship care, lead tenant, independent living or within the family home.
