Your new role

You'll be part of a dynamic team delivering a wide range of programs across HACC, Support for Carers, and other community initiatives. We work flexibly, creatively, and collaboratively to support people experiencing changing or emerging needs — always with a strong focus on inclusion, access, and community connection.

In this role, you’ll provide person‑centred, early‑intervention support to individuals, carers, and families. Your days might include:

  • Conducting assessments and offering targeted short‑term support
  • Goal‑based planning and coaching
  • Linking clients with specialist services
  • Delivering group programs and practical day‑to‑day assistance
  • Supporting carers to stay connected, confident, and resilient

You're someone who thrives when supporting others — and you bring warmth, initiative, and sound judgement to every interaction. You will enjoy this role if you are:

A natural relationship builder — empathetic, approachable, and able to connect with people from diverse backgrounds.
Confident working independently — you’re comfortable making thoughtful decisions and prioritising what matters.
Organised and dependable — able to manage competing needs while staying within program guidelines.
A clear communicator — skilled in coordination, collaboration, and keeping people in the loop.

What Makes This Role Meaningful

You’ll meaningfully improve everyday life for carers, younger people, and community members.
You’ll have autonomy — backed by a supportive and experienced multidisciplinary team.
You’ll enjoy varied, community‑based work that keeps you connected, engaged, and constantly learning.

If you're passionate about strengthening community connections and supporting people to live well, we’d love to hear from you.

Apply now and help create stronger, more connected lives.
