Your new role

As an Intensive Case Manager, you will deliver the Journey to Social Inclusion (J2SI) program, providing long-term, personalised support to people experiencing chronic homelessness. Working with a small caseload, you’ll use a trauma-informed, team-based approach to help clients access housing, build life skills, and foster independence. J2SI is delivered by a consortium of community organisations and funded by the Victorian Government.

Your skills and experience
  • Qualification relevant to the delivery of community services e.g. Social Work, Psychology, Occupational Therapy or equivalent. Candidates with Diploma qualifications and outstanding relevant experience may be considered.
  • Valid driver’s licence to drive in Australia.
  • Demonstrated experience in case management
  • Proven ability to restore relationships with people experiencing vulnerabilities and history of complexities in areas such as adult homelessness, alcohol and other drugs, mental health, disability, and family violence. 
  • Ability to work effectively in an outreach capacity
  • Sound knowledge of various theoretical frameworks and strategies applicable to working with clients with complex needs.
