Integrated Family Services Worker – Children with Complex Disability Needs

  • Help families with disability needs stay together.
  • Connect families to services, ensuring they have necessary supports in place.
  • SCHADS Level 4, Maximum term, full time/part time to June 2025, based in Horsham.

Work with families and services to prevent children from requiring care outside the family home because of complex disability support needs.

Your expertise will enable parents to navigate the National Disability Insurance Scheme (NDIS) and learn how to advocate for plans that contain disability supports that will enable them to provide care within the home, or support family reunification.

Providing advice and guidance to the family you will develop and maintain professional relationships with providers and work collaboratively to identify the disability support needs of the family.

 Your skills and experience

You will bring 

  • Tertiary Qualifications in human services, welfare, social work and/or relevant experience preferred but not essential.
  • Driver’s License.
  • Working with Children Check. 
  • Satisfactory Criminal Records Check. 
  • Demonstrated ability as a practitioner and case manager in the disability, child, youth, and family welfare field.
  • Strong case management experience.
