As part of our multidisciplinary Carers Independent Living Support team, you’ll deliver flexible, person-centred support across a range of programs including HACC and carer support services, alongside other community-based initiatives.
You’ll work with people from diverse backgrounds, helping them navigate changing needs, stay connected, and build independence. This is a dynamic role that combines early intervention, practical support, and community engagement, all with a strong focus on inclusion and wellbeing.

A relevant qualification in Social Work, Community Services, Psychology, Counselling, OR a related discipline, OR demonstrated equivalent knowledge and experience relevant to community services practice.
