Join our dedicated team committed to making a positive impact on the lives of individuals under 65 years through the Home and Community Care Program for Younger People (HACC PYP). We believe in a holistic, person-centered approach to enhance the quality of life and promote independence.

As a Flexible Support Coordinator, you will play a key role in implementing our mission by providing active service coordination for individuals requiring assistance in daily living. This position focuses on fostering independence, improving social connectedness, and enhancing emotional well-being through a range of capacity-building and social inclusion activities. You will lead a team of volunteers to create a supportive environment that promotes human dignity and upholds principles of social justice and equity.

Your skills and experience

You will meet the person specification in the position description including having a relevant tertiary/post-secondary qualification in community services, disability, or related disciplines. Experience of working with vulnerable people would be advantageous.

You will also have:

  • Demonstrated ability to participate in a multi-skilling learning environment and be able to relate positively to a broad range of people from diverse backgrounds.
  • Ability to deliver capacity building and social inclusion activities for people with all abilities.
  • Knowledge of and previous working experience in the disability field.
  • Experience in advocating for disadvantaged individuals with complex needs.
  • Experience in coordinating and supporting Volunteers.
