In this role, you will be conducting comprehensive, holistic in-home assessments, ensuring that individuals receive tailored support that meets their unique needs. Beyond assessment, you'll play a pivotal role in connecting clients with a network of both funded and non-funded services, fostering a collaborative approach to holistic care.
Additionally, you'll have the opportunity to arrange brokered services, facilitating access to specialised support when necessary. This role is not just about ticking boxes; it's about creating meaningful connections, advocating for those in need, and empowering individuals to thrive within their communities. About the programs
Home and Community Care Program for Younger People (HACC-PYP) is for people aged under 65 and Aboriginal people aged under 50 who need assistance with daily activities. The program has been designed to support people with disabilities, a medical or health condition, and their respective carers to live as independently as possible in their own homes.
The Support for Carers Program (SCP) is funded by the Victorian Government for unpaid carers. An unpaid carer is a person providing care to another person with care needs. A carer may look after a partner, family member, friend or someone else who needs support. Carers can be of any age, from any culture or country, and any socio-economic background.
