We have an exciting opportunity for a passionate Youth Case Manager to join our Homelessness Support Services Program. The Youth Homelessness team delivers early intervention transitional support to young people and couples aged 15-25, and their young families who are homeless or at risk of experiencing homelessness.
As a Youth Case Manager, you will provide engaging case management support to clients from a diverse range of backgrounds, helping them to break the cycle of homelessness. The role will assist clients to identify and address their individual support needs that contributed to their homelessness. Working as part of a committed team, the Youth Case Manager will respond to referrals in a timely manner, engage young people through outreach, and support them to access relevant specialist services and programs.

In addition to your starting base hourly rate of (SCHADS Level 4) + super, you can access the following.
