The Active Hold Practitioner position within the Navigator Program involves supporting young people aged 12-17 who are disengaged from education, providing crucial assistance during the "active hold" phase before they receive full case management services. This role requires the practitioner to engage with young people and their families through various communication methods, ensuring at least fortnightly contact, and assessing their needs and risks. The practitioner will support up to 55 young people, triaging cases, identifying immediate opportunities for support, and coordinating with schools and other key stakeholders to facilitate re-engagement in education.

You should have a relevant tertiary qualification in Social Work, Psychology, Child Development/Community Welfare, or a similar field, along with a current Victorian driver's license.
