Your new role

The Active Hold Practitioner position within the Navigator Program involves supporting young people aged 12-17 who are disengaged from education, providing crucial assistance during the "active hold" phase before they receive full case management services. This role requires the practitioner to engage with young people and their families through various communication methods, ensuring at least fortnightly contact, and assessing their needs and risks. The practitioner will support up to 55 young people, triaging cases, identifying immediate opportunities for support, and coordinating with schools and other key stakeholders to facilitate re-engagement in education.

Key responsibilities include:

  • Conducting ongoing risk and needs assessments.
  • Maintaining accurate case records.
  • Adhering to program guidelines.
  • Ensuring effective communication and documentation within the DE and Navigator Data Management System.
  • Fostering relationships with schools, education providers, and other community partners to ensure the best outcomes for the young people in the program.

Your skills and experience

You should have a relevant tertiary qualification in Social Work, Psychology, Child Development/Community Welfare, or a similar field, along with a current Victorian driver's license.

You will also have:

  • Experience working with young people and families impacted by mental health issues, substance misuse, and family violence.
  • A strong understanding of the Child Protection and Out of Home Care systems, experience in risk assessment, and a demonstrated ability to engage with schools and vocational training providers.
  • Experience in developing and implementing effective interventions, with a sound understanding of child and adolescent development.
  • Knowledge of culturally safe practices for diverse communities, including Aboriginal, Torres Strait Islander, CALD, and LGBTIQ+ groups, is crucial.
  • Strong communication skills, with the ability to collaborate effectively with other professionals and stakeholders.
