About the opportunity

The Home-Based Care Case Manager is responsible for delivering high quality support and case management. The position works with and supports carers and children/young people who are unable to live at home with their families using a therapeutic approach and the Looking After Children case management framework.

The position will case manage children/young people on statutory orders who are placed in home-based care within the Wimmera region ensuring they and their carers are safe and supported. The position is also responsible for delivering goals relating to Targeted Care Packages as outlined in the proposal and funding documents for each individual package where applicable.

About you

To be successful in this role, you will have:

  • Relevant post-secondary qualifications in Social Work, Welfare Work or Social Sciences, or less formal qualifications with significant industry experience (preferred but not essential).
  • Experience working with children and families in the child protection area.
  • Experience in working effectively in a care team situation and the ability to manage complex relationships and interactions.
  • Experience in supporting young people to grow and develop.
  • Experience in working with traumatised young people.
  • Providing case management within complex family circumstances where multiple service providers may be involved and/or there are complex family issues.
